What We Do: The service will provide safe, caring, therapeutic, short breaks to children/young people who have significant health care needs which give them a positive experience whilst providing their families with a break. This will include short breaks in the child/young person’s home or in the specialist unit. The breaks will reflect the individual therapeutic and social needs of thechild/young person and be provided in the context of their care plan.

Services Offered: Pre-School Children

The service will provide a range of short breaks for Pre-school children. This may include a weekly session of Day Care at the centre or a range of home based short break sessions. These short breaks sessions will include therapeutic and play activities with an individualised programme of care to meet the feeding and health care needs of the child. The service to preschool children will cease when they become eligible for an educational placement, normally from the age of 2 years and 9 months to 3 years. Preschool children will be offered a multidisciplinary (everyone working with the child from any service) review including the parents twice a year and this information may be used as part of the assessment of the child’s educational needs in forward planning.

School Age Children The service will provide a range of short breaks for school age children and young people. This may include overnight stays during the week from after school hours in the afternoon until school transport or parent/carer arrives in the morning, regular weekend stays from Friday afternoon until Monday morning and planned home short breaks sessions typically 4 hrs long on a fortnightly basis.
